This Notice describes how your protected health information (PHI) may be used and disclosed, and how you may access it.

 

Your HIPAA Rights

You have the right to:

• Request access to your medical records

• Request corrections

• Ask for confidential communication methods

• Request restrictions on certain uses

• Obtain an accounting of disclosures

• Receive a copy of this Notice

• File a complaint without retaliation

 

Our HIPAA Responsibilities

RevIVe must:

• Maintain PHI confidentiality

• Provide this Notice

• Notify you of breaches involving unsecured PHI

• Follow current HIPAA laws

 

Permitted Uses & Disclosures

1. Treatment

Sharing PHI with other providers involved in your care.

2. Payment

Insurance billing (if applicable), benefit verification, claim processing.

3. Healthcare Operations

Quality assessment, training, internal administration, accreditation.

 

When PHI May Be Disclosed Without Authorization

• Public health reporting

• Abuse, neglect, or domestic violence

• Court orders

• Law enforcement

• Serious threats to health or safety

• Workers’ compensation

• Health oversight agencies

 

Uses Requiring Authorization

You must authorize:

• Marketing not related to your treatment

• Sales of PHI

• Most uses of psychotherapy notes

You may revoke authorization anytime.
